If you want to submit a completely new profile or if you have not yet competed a profile before, go to the Oklahoma Author Survey. Download it to your computer and open it from the location you saved it on your computer. Your information will not be saved if filled out in the browser. Email completed form to info@libraries.ok.gov with subject: Oklahoma Authors.
