Records Destruction

Agencies must notify Administrative Archivist Jan Davis at least thirty (30) days prior to any proposed destruction of records by submitting one of the following forms:

ARC Form 4 (for paper)—ARC Form 5 (for microfilm)—ARC Form 6 (for microform)

You may destroy records only if the destruction is approved, and no legal action is pending.

For records scheduled to be destroyed on a regular basis (daily, weekly, monthly or quarterly) agencies may submit ARC Form 4 asking the Administrative Archivist to authorize destruction without submitting advance notification.  If the request is approved, you must report the volume of records destroyed at the end of the fiscal year.

Agencies that store records with the State Records Center will receive ARC Form 3 when these records are eligible for destruction.  The agency has thirty (30) days to review the form.  The records are not destroyed until this form is signed by the agency and approved by the Administrative Archivist.
